OneEighty’s health care profiles: Porchlight VNA rehab
Pat Cassetta, of Westfield, is a self-made man on the move. He has had a rewarding career in the paper industry, owning and partnering in several successful ventures. Today, he continues to work out of his home where he loves tending his garden of tomatoes and spices that flavor his homemade specialty dishes like pasta faggioli and roast pork.
On July 17 Mr. Cassetta fell and fractured his pelvis. After a short stay at Bay State Health Medical Center and a consultation with his primary care physician Jeffrey Cossin, MD, he went to Loomis Lakeside at Reeds Landing for rehabilitation. He received physical and occupational therapy. On July 31, he was discharged to home, but he would need the support of Porchlight VNA to complete his rehabilitation and regain independence.
Porchlight’s Interdisciplinary Services
Porchlight’s experienced Physical Therapists (PTs) and Registered Nurses (RNs) developed a plan for Mr. Cassetta to regain his strength and full independence in his home.

Porchlight’s registered nurses focused on pain management, wound care and education.With his pain under control, Porchlight’s Therapy Team was able to design an individualized home exercise program.

PTs guided him through a variety of exercises with a focus on transfers, balance and gait. He progressed in ambulating with the use of a walker to a cane independently.
Outcome: After seven weeks with Porchlight, Mr. Cassetta progressed from ambulating with the assistance of a walker to a cane, and now, to walking without assistance. Being fully independent means he can get back to running his business and back to his kitchen to cook he and his wife’s favorite meals.
